My office is located at 1300 West Belmont Avenue, in the Lakeview Neighborhood of Chicago, at the intersection of Belmont and Lakewood Avenue. Come to the side entrance on Lakewood Avenue and, if during normal business hours, enter the building and take the elevators to the 5th floor.
If the door is locked (ie; you are arriving after-hours), you will need to enter the current Security Code for the door entry intercom system. Dr. Mills will provide this code to you in advance of your appointment. Please enter this code in your cell phone for future reference, but please note the entry code changes periodically. If you do not have the code, please follow the instructions at the intercom to page Matthew R. Mills, M.D. in Suite 504.
Once arriving on the 5th floor, please exit the elevators and walk down the hallway to the last suite on the left. Please have a seat in our very comfortable waiting room and I will come to greet you at your appointment start time.

From the North:
  1. Take 90/94 East (Dan Ryan) to the Belmont Avenue Exit.
  2. Turn left onto Belmont Avenue and go East until you reach Lakewood Avenue.
  3. Parking options on Belmont Avenue as well as in the immediate neighborhood.
From the West:
  1. Take I-88 East to 290 East (Eisenhower Expressway).
  2. Take 290 East to 90/94 West (Kennedy Expressway).
  3. Take 90/94 West to the Belmont Avenue Exit.
  4. Turn right onto Belmont Avenue and go East until you reach Lakewood Avenue.
  5. Parking options on Belmont Avenue as well as in the immediate neighborhood.
From the South:
  1. Take I-55 North or I-57 North to 90/94 West (Kennedy Expressway).
  2. Take 90/94 West to the Belmont Avenue Exit.
  3. Turn right onto Belmont Avenue and go East until you reach Lakewood Avenue.
  4. Parking options on Belmont Avenue as well as in the immediate neighborhood.
Public Transportation
From the Red / Brown / Purple Line (CTA):
  1. Exit at the Belmont Avenue Station (945 W. Belmont Ave).
  2. Walk 0.4 mile West to 1300 West Belmont Avenue.
From the Brown Line (CTA):
  1. Exit at the Southport Avenue Station ( 3411 N. Southport Ave).
  2. Walk 0.3 mile South to West Belmont Avenue.
  3. Turn left at W. Belmont Avenue and walk 1 block West to Lakewood Avenue.

  • 'Pay For Park' options on W. Belmont Avenue directly in front of the building.
  • Also, there is non-permit residential street parking in the immediate area, South of Belmont Avenue.
  • Finally, there are non-metered parking spots open on occasion on Belmont Avenue directly across from the building.
